What belongs in a reading practice log you would actually trust?

A log of practice between sessions is only worth keeping if the person reading it can trust what it says. Too little and it says nothing; too much and nobody fills it in.

For the tutors, teachers and clinicians here: what has to be on a practice log before you would act on it? The skill practiced, the minutes, first tries versus repeats, an honest note on how it went, examples of the errors themselves?
